- A statement that the information in the notice is accurate and that you are authorized to act on behalf of the copyright owner under penalty of perjury.
Please note that under 17 U.S.C. §512(f), you may be liable for damages (including costs and attorney’s fees) if you knowingly make false claims of copyright infringement.

By submitting a complaint, you acknowledge and agree that we may forward your notice and contact information to the alleged infringer.
Counter-Notice – Restoration of Material
If you believe that material was removed or disabled by mistake or misidentification, you may submit a counter-notice.
Under 17 U.S.C. §512(g)(3), your counter-notice must include:
- Your physical or electronic signature;
- A description of the removed content and its original location;
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled due to an error or misidentification;
- Your name, address, phone number, and a statement that you consent to the jurisdiction of the federal court in your district (or any U.S. federal court if you are outside the U.S.) and accept service of process from the person who submitted the original DMCA notice.
